HCR Home Care (“HCR,” “we,” or “us”) is committed to protecting your privacy and security. This Privacy Policy describes our practices for information collected through the Services and does not replace any separate agreements, authorizations, or notices that may apply to HCR’s provision of home health care services.

HCR offers a full spectrum of home health care and social services customized to meet clients’ unique needs in the comfort of their homes. HCR may enter into separate written agreements with clients, patients, authorized representatives, or other parties in connection with those services.

Protected health information (“PHI”) that HCR creates, receives, maintains, or transmits in connection with home health care services may be governed by the Health Insurance Portability and Accountability Act (“HIPAA”), HCR’s Notice of Privacy Practices, separate agreements, authorizations, and applicable law.

Please do not submit PHI or other sensitive health information through general Site forms unless HCR specifically directs you to do so. The Site is not intended for medical emergencies or urgent care communications; in an emergency, call 911 or use the emergency contact procedures provided by HCR.
