- 1978 – HCR Home Care is established in Rochester, NY and employs 3 people
- 1979 – HCR introduces Shared Aide concept
- 1982 – HCR installs our first computers
- 1984 – HCR employs over 100 people
- 1987 – HCR is #1 in Monroe County, New York, by Health System Agency Rating
- 1988 – HCR becomes a Certified Home Health Agency (CHHA)
- 1996 – HCR’s first full-scale computer network is installed
- 1997 – HCR named “Employer of the Year” by Private Industry Council of Rochester (NY)
- 1999 – HCR included in Democrat and Chronicle’s “Rochester’s Best Place to Work” list
- 2002 – Health Grades names HCR the Top Quality Provider in Monroe County (NY) with a five star rating
- 2003 – Migration to MISYS/AllScripts software completed
- 2005 – Total Knee Replacement protocol study completed; HCR and MVP partner in promoting new protocol to area orthopedists
- 2006 – HCR becomes an employee-owned company
- 2006 – The Rochester Business Alliance awards HCR the 2006 Ethics Award for large businesses
- 2008 – HCR receives the IPRO Quality Award for the 2nd consecutive year
- 2008 – HCR awarded the Leininger Breakthrough Honorable Mention Award by the Transcultural Nursing Society
- 2009 – Nursing Outlook Magazine published the peer-reviewed results of HCR’s first Transcultural care initiative, Project Exito!
- 2010 – HCR, now with over 900 employees, officially begins operations in Genesee, Schoharie, and Orleans Counties
- 2011 – HCR named Top 100 agency by HomeCare Elite
- 2011 – HCR begins serving patients in Cortland, Delaware, and Madison Counties
- 2012 – HCR begins serving patients in Clinton County
- 2013 – HCR begins serving patients in Otsego and Washington Counties
- 2014 – HCR begins serving patients in Cayuga, Essex, Franklin, Hamilton, Jefferson, Onondaga, Oswego, St. Lawrence, and Warren Counties
- 2015 – HCR begins serving patients in Livingston, Ontario, and Wayne Counties
