Salary Range: $30.05 To $34.86 Hourly


Role and Responsibilities 


Administer direct skilled nursing care to patients, in accordance with physician’s orders. 


Essential Functions 

  • Provide hands-on, skilled nursing care in accordance with physician’s orders. 

  • Provide health education to the patient and family. 

  • Assess patient response to the plan of care interventions and communicate changes to the case manager. 

  • Collaborate and share findings with the multidisciplinary care team. 

  • Document those services rendered to the patient, note changes in the patient’s condition and/or family and home situations.  

  • Document communication with case manager and physician. 

  • Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager. 

  • Other duties as assigned.

Education Requirements 

  • Current, valid RN license issued by NYS Department of Education.

 

Qualifications and Requirements 

  • Community health experience preferred. 

  • Valid and unexpired NYS Driver’s License. 

  • Current CPR certification. 

  • Strong communication and documentation skills. 

  • Critical thinking skills, and ability to function autonomously.  

  • Clinical Competency of nursing skills. 

  • Works as team player and collaborates with the patient care team. 

Work Environment 

The RN Procedure Nurse is primarily in a non-office setting and may be exposed to outdoor conditions. 


The working conditions are classified as medium work: 

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing. 

Physical Requirements
