Role and ResponsibilitiesÂ
Administer direct skilled nursing care to patients, in accordance with physician’s orders.Â
Essential FunctionsÂ
Provide hands-on, skilled nursing care in accordance with physician’s orders.Â
Provide health education to the patient and family.Â
Assess patient response to the plan of care interventions and communicate changes to the case manager.Â
Collaborate and share findings with the multidisciplinary care team.Â
Document those services rendered to the patient, note changes in the patient’s condition and/or family and home situations. Â
Document communication with case manager and physician.Â
Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager.Â
Other duties as assigned.Â

Education RequirementsÂ
Current, valid RN license issued by NYS Department of Education.
Â
Qualifications and RequirementsÂ
Community health experience preferred.Â
Valid and unexpired NYS Driver’s License.Â
Current CPRÂ certification.Â
Strong communication and documentation skills.Â
Critical thinking skills, and ability to function autonomously. Â
Clinical Competency of nursing skills.Â
Works as team player and collaborates with the patient care team.Â
Work EnvironmentÂ
The RN Procedure Nurse is primarily in a non-office setting and may be exposed to outdoor conditions.Â
The working conditions are classified as medium work:Â
Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.Â
