About HCR Cares

The desire for independence never grows old, which is why quality home-based health care that is safe and successful is so important. At HCR Cares, our research and educational activities focus attention and advance knowledge in key aspects related to independent living. As we foster best home care practices and identify new ways to serve those whose independence is challenged, available services are enhanced.

Mission Statement

All individuals can remain in their home safely and successfully as long as they choose.

Nursing Grants

HCR Cares offers several grants each year to individuals who would like to pursue a degree in nursing ranging. The grants aim to strengthen and diversify the nursing work force by offering opportunities to ambitious, hardworking, and qualified individuals who wish to advance in the field of nursing. The nursing grants are meant to help with extra and/or unexpected expenses, beyond tuition, which students encounter when they go back to school, e.g. books, extra daycare, technology, licensing fees, or income supplement. In addition to money, grantees are mentored by Board members, who help them with financial planning, studying techniques, and preparation for licensing exams.

Can We Talk

Can We Talk 
is a workshop designed for LGBTQ patients and health care providers to share and learn from each other how to better provide quality healthcare to LGBTQ patients. The workshop has two main parts. The first part has LGBTQ patients sitting in an inner circle, sharing stories and issues they encounter as patients. The health care providers sit in an outer circle and listen. The second part has health care providers and patients switching circles and roles. Both parts are led by experienced facilitators. The goal of the workshop is for both patients and providers to leave with a better understanding of the issues and sensitivities involved in providing quality health care to the LGBTQ community.

Falls Prevention

One area of particular focus for HCR Cares is the prevention of falls among older individuals.  According to the Centers for Disease Control and Prevention (CDC), an older adult is treated in a hospital emergency room for a fall every 18 seconds. Every 35 minutes an older adult dies as a result of a fall-related injury. Statistics on the costs of falls are alarming:

In New York State, about 1.7% of people age 65 and older, or more than 42,000 people, were hospitalized through the ER because of a fall.

If the current rate of falls among seniors is not reduced, fall-related treatment costs will reach $43.8 billion annually by 2020, according to estimates by the CDC.
