Nursing that goes beyond the bedside

Why Choose Us?

At HCR Home Care, RNs aren’t one nurse among many on a floor. You own your caseload, lead the care team, and make the kind of difference that only happens when you meet a patient in their own home.

  • 35+ Years serving New York State

  • 6 Regional offices across NYS

  • 100% Employee-owned (ESOP)

  • M–F — Primarily daytime field schedule

Top Workplace — Multiple Years | Recognized by employees, not just awards committees.

"As a nurse at HCR Home Care, I truly value the autonomy to care for my patients, paired with the assurance that support is readily available whenever I need guidance or assistance. I deeply appreciate the strong, collaborative relationships I have built with my coworkers and management.
- Tish D., RN Case Manager

Ready to lead patient care where it matters most?

Join HCR’s team of dedicated home care nurses and build a nursing career you’re proud of.

Pillar 1: Own your patient relationships

You’re the clinical lead on your caseload — building continuity of care and trust with patients and families over time, not just at handoff.

Pillar 2: Flexible, field-based schedule

Manage your own caseload with autonomy and flexibility. Primarily Monday–Friday daytime visits — no rigid hospital shift rotations.

Pillar 3: Lead the full care team

As RN Case Manager, you direct and supervise LPNs and Home Health Aides — practicing true clinical leadership on every case you manage.

Pillar 4: State-of-the-art technology

Point-of-care documentation and real-time patient data tools mean less time charting and more time focused on what matters — your patients.

Pillar 5: Clinical specialization opportunities

Develop expertise in cardiac care, diabetes management, wound care, falls prevention, and more through HCR’s specialized home care programs.

Pillar 6: Employee ownership (ESOP)

Every HCR employee is an owner. Our Employee Stock Ownership Plan builds your retirement as the company grows — a benefit few employers offer.

A nursing career built on autonomy, impact, and ownership

Home care nursing is one of the most rewarding — and most autonomous — settings in the profession. HCR gives you the tools, team, and culture to do your best clinical work.

What your day as an HCR RN Case Manager looks like

Step 1: Admissions & Assessments

Conduct comprehensive in-home admission assessments — medical history, physical examination, mental status, ADLs, environment, and family support — to build a complete patient picture.

Step 2: Develop & Manage the Plan of Care

Create individualized care plans per physician orders, update them as patient conditions evolve, and maintain overall accountability for outcomes — including care delivered by LPNs and HHAs.

Step 3: Skilled Nursing Visits

Deliver skilled clinical care in the home — wound care, IV therapy, medication management, vital sign monitoring, and more — while teaching patients and families at every step.

Step 4: Coordinate Interdisciplinary Care

Collaborate with physicians, therapists, social workers, and aides to ensure every aspect of a patient’s care is aligned and communicated in real time.

Step 5: Document, Supervise & Discharge

Complete accurate point-of-care documentation, conduct aide supervisory visits, manage cost-effective care delivery, and plan discharges that set patients up for long-term success.
