What your day as an HCR RN Case Manager looks like
Step 1: Admissions & Assessments
Conduct comprehensive in-home admission assessments — medical history, physical examination, mental status, ADLs, environment, and family support — to build a complete patient picture.
Step 2: Develop & Manage the Plan of Care
Create individualized care plans per physician orders, update them as patient conditions evolve, and maintain overall accountability for outcomes — including care delivered by LPNs and HHAs.
Step 3: Skilled Nursing Visits
Deliver skilled clinical care in the home — wound care, IV therapy, medication management, vital sign monitoring, and more — while teaching patients and families at every step.
Step 4: Coordinate Interdisciplinary Care
Collaborate with physicians, therapists, social workers, and aides to ensure every aspect of a patient’s care is aligned and communicated in real time.
Step 5: Document, Supervise & Discharge
Complete accurate point-of-care documentation, conduct aide supervisory visits, manage cost-effective care delivery, and plan discharges that set patients up for long-term success.

What we look for in an HCR RN

Education & Licensure
-
Associate’s degree in Nursing required; BSN preferred

Current, valid RN license issued by NYS Department of Education

CPR/BLS certification
