HCR’s Transcultural Program is based on Madeleine Leininger’s Theory of Culture Care Diversity and Universality. Our culture-based home care models specifically address Hispanic, African American, and Ukrainian cultures. Transcultural care is especially appropriate for rural/farm communities and the unique culture characterized in these areas.

HCR will respect and support your family traditions, religious beliefs, and facilitate your language requirements. Bilingual staff provide the expert care you expect and deserve. Educational materials and other printed directives are available in Spanish, Ukrainian, and Russian. HCR also understands and supports unique food traditions. Our diabetic and heart-healthy meal plans have been thoughtfully developed according to your cultural preferences.

LGBTQ+ Care Program

HCR Home Care established the Lesbian, Gay, Bisexual, Transgender, and Queer/Questioning Care Program in order to promote the health and well-being of LGBTQ+ patients served in Rochester and Monroe County. Through the delivery of high quality, LGBTQ+-Affirming Home Health Care services, we are dedicated to providing safe and inclusive environments to help our patients thrive and meet their health care goals.

LGBTQ+ individuals are more likely to delay seeking care out of fear of discrimination, and are at higher health risks of obesity, diabetes, social isolation and depression which puts them at greater risk of further health complications. Our dedicated multidisciplinary home health care staff is certified and trained in LGBTQ+ Cultural Competency by Out Alliance (https://outalliance.org/), which helps equip our staff with the tools and knowledge to meet the specific needs of our LGBTQ+ patients.

HCR is committed to providing safe and competent care without prejudice or discrimination of race, color, religion, creed, gender, national origin, disability, sexual orientation, marital or veteran status, age, gender identity, or any other legally protected status. HCR Home Care and HCR Care Management advocate services for transgender patients. We help transgender individuals navigate the process to change identity, and connect individuals with community resources.
