Home Health Aides
Home Health Aides work under the direction of our nurses and therapists. They assist with daily living activities, such as bathing, dressing, meal preparation, laundry and ironing, coordinating appointments, errands and shopping, care and feeding of gentle pets, offering assistance at outings and functions, providing companionship, or just about any care you need to stay comfortable, safe, and happy at home.
Geriatric Care Managers
Geriatric Care Managers can help you to better understand your loved one's needs; identify and recommend appropriate services and programs; provide guidance on long term planning, housing options, and alternative types of care; and consult on costs and coverage associated with insurance options. GCM's can also serve as the “eyes and ears" of family members who live far from their parent or loved one.
Social Workers
Medical Social Workers help find community resources, assist with insurance and financial paperwork, address long term care needs, and provide emotional support.
