As a veteran, you’ve served our country in a way only a select few Americans can claim. You’ve fought for our freedoms and interests worldwide, and did it with honor and dignity. The compassionate caregivers at HCR appreciate the sacrifices you and your family have made. We are grateful for your service and want to provide you with the support you need at this stage of your life and beyond.

HCR recognizes the unique needs of our distinguished veterans, and has taken the steps necessary to create a veteran-friendly home care environment… one that provides you with the best quality home care. Our skilled health care professionals offer expert home care to veterans with all types of health-related needs, including medical and surgical care, chronic disease care, and pain management.

In addition, HCR provides a full complement of home health aide services. Working under the direction of our nurses and therapists, home health aides assist our veterans with activities of daily living, such as bathing, dressing, meal preparation, laundry, coordinating appointments, running errands, and providing companionship. The goal of the home health aide is to help keep you comfortable, safe, and happy at home.

In Touch with the VA

The clinical care coordinators at HCR work in partnership with Veteran’s Administration personnel to help maximize your VA and Medicare benefits – ensuring you receive the full range of services to which you are entitled. The close relationship HCR has with the VA and regional VA health centers helps provide peace-of-mind for our veterans, and allows them to focus on improving their wellness.

The leadership at HCR meets regularly with veteran advocates to help us better understand the needs of vets in our area – from World War II, the Korean Conflict, and Viet Nam… to the Gulf Conflict and our current involvement in Afghanistan and Iraq. As always, we support the work performed by organizations such as Disabled American Veterans (DAV), the American Legion, Veterans of Foreign Wars (VFW), and Veterans Outreach Center (VOC).

Arranging Home Care Services is Easy

Home care services are often recommended by your VA discharge planner, physician, or by a social worker at the hospital. These individuals will generally initiate your referral to HCR. However, you may contact HCR at any time and ask for assistance in starting services. Contact our Care Coordination department either on-line, or by calling 585-272-1901.
