The Centers for Medicare & Medicaid Services (CMS), an agency of the US Department of Health and Human Services, in conjunction with the Home Health Quality Improvement Organization Support Center (HHQIOSC), has launched the Home Health Quality Improvement (HHQI) National Campaign. The HHQI National Campaign seeks to unite the home care community under the shared vision of reducing avoidable hospitalizations to improve patient quality of care
Recognized by the HHQI National Campaign as having Premier Status, HCR is dedicated to providing the best quality care for its patients, and has been able to achieve outcomes that surpass the National and New York State averages.
